OHIO – The Ohio Department of Job and Family Services has a page that allows employers to report when a employee has failed to return back to work after COVID-19 closings.

This report on the website says, “Employers may use the following form to report employees who quit or refuse work when it is available due to COVID-19.”

Employers will have to fill out needed information on the report including when the store closed, if the store was deemed a essential business and the employees Social Security number.

ODJFS says then, “We will review the necessary files and records in light of the information you have provided to determine the most appropriate action.”
