Jason Mathew Taylor, 35, of Circleville, died May 3, 2015, at his residence.
He was born Dec. 9, 1979, in Columbus, the son of Joseph and Constance (Clark) Taylor.
He was preceded in death by his father.
He is survived by his mother; children, Mason, Anthony, Deanne and Destiny; nieces, Samantha, Hailey and Maranda; cousin, James Taylor; sister, Tressa Imler; and several family and friends.
Memorial service will be held at 1 p.m. Saturday, May 9, 2015, at the Wellman Funeral Home, Circleville with Rev. James Taylor officiating.
